Clinical History-
54-year-old male with abdominal pain and obstructive jaundice. Underwent EUS guided biopsy from the head mass.
From the images below identify the likely disease.
On endoscopy-
EUS- showed mass in head of pancreas
Microscopic Images-
Histology images showed small bowel mucosa with presence of a neoplasm composed sheets of cells.
Immunohistochemistry Performed-
Immunohistochemistry performed showed the atypical cells are negative on Synaptophysin, Chromogranin, CK7, TTF1, CDX2 with Ki67 index of 40-50%. IHC was positive for CK20 and GATA 3

Metastasis to the pancreas is a rare cause of pancreatic masses.Most pancreatic tumours are primary. Metastasis cancers to pancreas are rare and usually come from lung, gastrointestinal tract and renal cell carcinoma. Metastatic Urothelial carcinoma to pancreas is extremely rare. Immunoprofile of GATA3, CK20, P63 aids in diagnosis.
