Interesting Case of the Month - (IGHP, ICOM) - May

Clinical History-

46-year-old obese female with height of 166cm and weight of 112 Kg and H/o NASH related CLD with features of decompensation including ascites, jaundice and HE underwent LDLT LFT Bili -3.0 mg/dl, SGOT/ SGPT-35/ 31, SAP/ GGT-92/ 19 and Protein/ Alb-6.2/ 2.4. ANA was positive (1:20)

Radiology-

HRCT -Showed features of chronic liver disease with splenomegaly and portosystemic collaterals.

Gross Image-

Expand liver cut surface showed regular margins and variegated greenish yellow cut surface.

Microscopic Examination -

Microscopic examination showed moderate to severe steatosis with cholangiolar cholestasis and areas of bridging necrosis

Special Stain Performed -

Masson’ trichrome stain showed pericellular and periportal fibrosis with bridging. alongside areas of multi acinar and bridging necrosis.

 

Immunohistochemistry -

IHC performed for CK8/ 18highlighted multiple Mallory Denk Bodies–
