Clinical History-

57-year-old female complaint of pain abdomen for 7-8 months, weight loss. From the images below identify the likely disease?

Radiology-

CECT-Diffusely bulky pancreas with peripancreatic halo ? AIP

Microscopic Images

Microscopic examination of the pancreatic biopsy showed benign pancreatic acini alongside large areas of fibrosis. The fibrosis has vague storiform arrangement of spindle cells . Mild periductal inflammatory infiltrate comprising oflymphoplasmacytic cells.

Special Stains Performed-

MT stain showed areas of fibrosis

Immunostains performed-

IHC performed for IgG4 showed 15-18 IgG4 positive cells / hpf

Diagnosis-

Adenocarcinoma Pancreas
Chronic pancreatitis
Metastatic carcinoma
Autoimmune Pancreatitis (AIP)
